Informed Consent/Hold Harmless Agreement for Sessions, Workshops and Classes

Agreement between “I” the participant and See Beyond Shadows, LLC

I understand that the Soul Lit Resonance techniques are complimentary and integrative healing modalities that in no way substitute for medical diagnosis, treatment or psychotherapy and it is my responsibility to consult a medical professional for diagnosis or treatment of any medical issue or injury. I understand that I am electing to participate in this and any subsequent sessions of my own volition. I understand that during the course of this work, the practitioner may make suggestions for additional practices and study as well as techniques which may have a neurological and anatomical impact.

The effects and benefits of this technique may vary depending on each individual’s response patterns.  Although there are no known harmful effects from this type of practice, I hold See Beyond Shadows LLC and the practitioner harmless from any possible effects that may cause physical or emotional discomfort and agree to take full responsibility for my self-care and personal development.  I am in control of my own body and I can always “stop” at any time.

I acknowledge that an open communication is promoted between me and the practitioner to enhance a mutual understanding and acceptance of the work provided during the session. I acknowledge that it is my responsibility to discuss medical issues or concerns with the practitioner in advance of the session.

I understand that there is a 48 hour notice policy for rescheduling/cancellation prior to the scheduled session. If I do not give ample notice, I will be charged the full price of the session. It is entirely at the discretion of the owner if I am awarded an exception to this policy.
